The Director of this operation provides care for children younger than 24 months of age, and does not have the required training. One clock hour of the annual training hours must cover the following topics: (1) Recognizing and preventing shaken baby syndrome and abusive head trauma; (2) Understanding and using safe sleep practices and preventing sudden infant death syndrome (SIDS); and (3) Understanding early childhood brain development.

The posted employee list was not current as it included names no longer associated with the opeation and did not list employees present at the operation.

One of the four children's files reviewed did not include a current immunization record.

A required individual was not added to the operation's controlling person list within the required time frame.

Two files reviewed did not include neceassy informaton for their listed emergency contact.

One of the four children's files reviewed did not include a signed health statement 12 months after the enrollment date.

The last gas inspection was conducted 12/13/2021.

At the time of inspection the changing table was not disinfected after a diaper change.

Upon arrival to the center, there was a drop gate that was not secured in the upward position with a child in the crib. This was corrected when the drop gate was secured in an upward position.

At the time of inspection the caregiver in the three year old classroom did not wash their hands after a diaper change. This was corrected when the caregiver washed their hands.

One child did not wash their hands after a diaper change. This was corrected when the caregiver washed the child's hands.

At the time of inspection a blanket was being used as a crib sheet for a child under 12 months.

At the time of inspection three infant feeding schedules were not available for review.

At the time of inspection, there was not any documentation for the fire drill being conducted this month. This was corrected when the documentation was correctly added.

At the time of inspection there were not any age appropriate toys in the classroom the children were combined in. The toys were only infant toys.

The annual fire inspection was expired.

At the time of inspection the diaper changing mat was not sanitized.

One infant was in the infant room unsupervised. This was corrected when the caregiver and the rest of the children entered the infant room.

The caregiver did not wash the child's hands after a diaper change. This was corrected when the child's hands were washed.

Two children older than 18 months were placed in the high chairs. This was corrected when the children were removed from the high chairs.

During inspection, there were seven children in care with one caregiver. There were two infants under eighteen months combined with five children between the ages of eighteen months and four years. According to minimum standards, there can only be four children or less age eighteen months to four years combined with two infants under eighteen months if there is only one caregiver present. This was corrected at inspection when another caregiver arrived and the operation was brought into ratio compliance.

A caregiver failed to wash one infant's hands after diaper changing.

Directors trainings reviewed did not have the following; Emergency Preparedness Food allergies, Understanding building and premises, Handling and storing and 6 hrs. of leadership. SIDS and communicable disease. 6 hrs. of leadership.

Three areas on the wall in the infant classroom were observed with peeled paint. An obvious odor lingered throughout the building that may due to gas or plumbing.

Four of the four children files did not have Special Care statement filled out on enrollment. Addition to the same four files not having Healthcare information. And two of the four not having Emergency Care Authorization and Facility care.
