Staff did not conduct required bed check, and two children ran away from the home.
07-17-2024. The assigned inspector conducted an on site unannounced follow up inspection and observed the operation to be clean and the bathrooms to be without standing water and there are new bathmats. The only hazard was wasps nests under the over hang at Tavern Oaks House.
06-06-2024. The assigned inspector conducted an unannounced on site follow up inspection. The bathroom tile is Tavern Oaks still has water on it because the edge of the shower has insufficient height to prevent water spreading to the floor during showers. A direct care staff cleaned the floor during the inspection. The hole in the Waddesdon Bluff bathroom wall is still there however the operation purchased a protective plate during the inspection. The operation has a meeting with a contractor on 06-18-2024 to repair paint chipping on the wall. The Tavern Oaks bathroom wall felt stronger and there was less of an odor in the bathroom. The inspector observed the outside areas of both houses clean with no brush. Air vents, tile and the kithcen cabinet were in compliance. In Waddeson Bluff Home the tile in the restr4oom was broke in the restroom and needed to be repaired. The wall in the first restroom had a hole that needed to be patched up. One of the bedrooms in the home needed calking in the ceiling. One of the faceplates needed a cover on it. The front door needs to be replaced. The home at Tavern Oaks needs to have front door replaced. There are places in the home the paint is peeling from that needs to be repaired. Branches in the back needs to be properly disposed of. The tile in the kitchen needs to be replace on the counter and the floor where it is broken. The air vents in the home need to be cleaned. The drawer under the sink needs to be repaired.

03-01-2024. The operation replied with a letter dated 06-09-2021 indicating annual testing is not required, however, did not provide documentation of any gas tests between 01-04-2021 and 01-04-2023. Standard left pending. The operation has until 03/03/2024 to provide documentation of a gas leak inspection and gas service from no later than 01-04-2023.

The operation provided confirmation documentation that they have a health inspection scheduled for 03-08-2024 but did not provide documentation of any health inspection completed between 11-28-2022 and 11-28-2023.
