During the investigation, it was determined that two car seats were not properly secured during transportation, resulting in injuries to two children.

Three of ten children records reviewed did not have current immunizations on file.

The operation did not have the name, address or telephone number of the local CCR office posted. Note: This was corrected at inspection when posting was placed near parent sign in area.

The operation discontinued services before contacting the parent to verify if their child had been withdrawn for the daycare. This resulted in the child not being transported from school to the operation.

The transportation log was not filled out for the children that were picked up from school and taken to the operation.

A five-month-old infant was sleeping in the crib with a blanket. The infant also had a pacifier with clip and holder attached to the infant?s clothing. Note: Caregiver in the infant classroom was the parent of the infant in the crib.

A four-month-old infant was sleeping on a bouncer seat.

There was a picket board detached from the fence line on the second play ground and a gap at the bottom of the chain link fence between the first and second play area.

The jungle gym dome did not have loose fill.

The jungle gym dome did not meet the required use zone on the playground.

There was one caregiver in the infant room with six infants. Note: This was corrected when a second caregiver walked in the room.

1 out of the 10 children's files reviewed did not have current immunization records.

There was no food allergy plan on file for a child enrolled that has a diagnosed food allergy.

It was determined that a caregiver inappropriately disciplined multiple children at the operation.

Recite: One staff records was available for review, all previous trainings were complete. One staff record was still pending for review.

Recite: 4 children's immunizations records previously cited were not ready for review. One children's immunizations was verified and accepted.

5 out of the 10 children's records reviewed were missing current immunization records.

Children in the pre-k room did not wash hands before having snack time.

2 out of the 10 staff records reviewed did not have all required trainings for previous training year on file. The trainings must cover the topics of food allergies, administration of medication, premises safety and handling/disposing of hazardous materials.

There were empty water bottles, broken hard plastic bottles and broken equipment on the infant playground

There were Clorox cleaning wipes and Febreeze sitting on top of the cubbies in both of the pre-k rooms within reach of children. Note: This was corrected at inspection when the caregivers moved them out of reach.

Caregivers in the pre-k room did not wash their hands before serving snacks to the children.

The fire extinguisher in the pre-k room was sitting on top of the shelf housing the children's cubbies and was not mounted to the wall.

2 children were missing infant feeding schedules in the Infant 2 room.

Both staff members who transport children did not have current transportation training on file.

3 out of the 4 children in the infant 2 room did not have bottles that were labeled with their first name and last initial.

There were no carbon monoxide tests documented for the current calender year.

1 of the cribs in the infant 1 room had a loose fitted sheet that presented an entaglement hazard.

Caregiver in the infant 2 room did not wash infant's hands after diapering.
