The operation discontinued services before contacting the parent to verify if their child had been withdrawn for the daycare. This resulted in the child not being transported from school to the operation.

There were Clorox cleaning wipes and Febreeze sitting on top of the cubbies in both of the pre-k rooms within reach of children. Note: This was corrected at inspection when the caregivers moved them out of reach.

The fire extinguisher in the pre-k room was sitting on top of the shelf housing the children's cubbies and was not mounted to the wall.
