The school aged playground had a broken plastic barrier, creating a sharp point on the play equipment. The playground also had tree limbs and broken play equipment in the play area.

On 11/14/24, the director was caring for 23 children with a specified age group of 4 years, placing the classroom over ratio by five children.

Child was sent home with feces in pants. Child was not asked if they needed to go to the restroom or checked by caregiver to see if they were clean prior to being sent home.

Parent was not notified when caregiver found blisters on a child's buttocks when being provided a diaper change in the morning. The child was later diagnosed to have Hand, Foot, and Mouth Disease.

The infant caregiver did not have the current SID's and Shaken Baby Syndrome traninings renewed.
