A castle slide was observed on the asphalt outdoor area. Information gathered during inspection states children play on the slide while being on the asphalt.

The diaper changing station in the toddler room underneath the mat had packages of construction paper, individual letters and strips of the alphabet. There was a purse belonging to the pre -k caregiver on top of a low shelf at hands reach of children. There were black trash bags filled with toys and equipment in the play area of the dinning hall used by children. There was a highchair in the dinning area that was torn from the upper area of the safety straps. The after school classroom had a table with a broken apron. There was a broken lid from a bin in the after school classroom. There was a pile of old boxes and trash bags towards the shed in the outdoor play area.

One child in care did not have signed Authorization for emergency medical attention.

A caregiver is missing 4 hours of annual training.

Based on information gathered, children were out of control when two children were able to hit another child with special needs who was hitting a caregiver. Parents present at the time of incident had to intervene when the caregiver could not redirect the children on their own.

Three employees did not report suspected abuse of a child after being told that a child sustained bruising from alleged inappropriate discipline by a caregiver.

An infant was observed sleeping with a blanket in a bouncer. This was corrected when the caregiver moved the infant out of the bouncer.

The transportation logs did not have emergency contacts or emergency care authroization forms for children being transported. This was corrected when the direcctor filed all the required information into the van log binders.

The diaper changing table was not cleaned or sanitized after it was used to change an infant's diaper.

Infant caregivers were observed not speaking or engaging with infants during diaper changing or while holding them.

A van that is used to transport children has a broken/jammed door that does not open.

Based on attendance sheets and employee timecards provided by the operation, multiple classrooms were out of ratio on the following dates: On March 7th, 2 caregivers were left responsible for 36 children including 7 infants between 6:30am-9:00am. On March 8th, 2 caregivers were left responsible for 45 children including 8 infants between 6:30am-9:00am. On March 9th, 2 caregivers were left responsible for 42 children including 6 infants between 6:30am-9:00am. On March 10, 2 caregivers were left responsible for 39 children including 7 infants between 6:30-9:00am. On March 11th, 3 caregivers were left responsible for 30 children including 6 infants between 6:30-9:00am.

Based on the information gathered during the investigation, a caregiver yells at children as a form of discipline.
