At time of inspection, operations last fire inspection was conducted on 11.4.24 that required corrections made and have not been completed.
Caregivers exhibited lack of judgement by failing to notify the parents and their administration of a child that exhibited and expressed signs of being injured. The child had been intermittently crying for from 850AM until 5PM . The child was later diagnosed with a dislocated and fractured finger.
Throughout the investigation, it was noted that caregivers did not assess the child when they were crying and holding their arm.

Three out of ten staff files reviewed were missing training over handling and storing of hazardous materials training.
A trash can in the Pre-School 1 classroom that had diapers in it did not have a lid making it accessible to children. This was CORRECTED AT INSPECTION when the trashcan was replaced with a new one that has a lid.
A child was on a device called the "spinning dummy" with another child and caregiver was not nearby for supervision.

The operation failed to submit a background check for a governing body designee tasked with overseeing day-to-day operations as the regional manager of the LLC that owns the operation.
Operations school-aged playground chain link was not properly secured in which allowed a child to exit the play area.
During inspection it was observed one out of the three buses used at operation, a seatbelt was not working.

She stated there were 8 children in her care when there were 9 in infant room and in toddler room she stated 16 and there were 17. The director corrected it during inspection by recounting children and rooster making sure all children were accounted for.
