The director had verification of only 8 hours of annual training completed in her file. One of five caregivers had verification of 14 of 24 hours completed of required annual training hours.

The PVC pipe around the playground was not installed properly, creating a tripping hazard.

A caregiver was on her phone while responsible for a group of children in care.

A mop bucket filled with water was in the classroom accessible to children in care. Note: Corrected at inspection when caregiver moved the bucket into the kitchen.

The pea gravel installed on the playground slide is not meeting the 6 inches of loose fill, as required by licensing.

Caregiver's lacked training in SIDS, Shaken Baby syndrome and understanding early childhood brain development.

The children's product form has not been re-certified, as required.
