The caregiver in the after school classroom gave the incorrect number of children in care in the classroom. Caregiver stated there were twenty-six children in care when there were twenty-five children in the classroom. Note: This was corrected at inspection when caregiver recounted the children and provided the correct number of children in their care.

During an investigation, it was determined a child exited their classroom when parent opened the door to the classroom. Child was unsupervised for approximately forty-five seconds until found.

Based on information gathered, caregiver was not actively supervising the children appropriately. Caregiver did not intervene when one child was on top of small bookshelf, and another child pushed the child who was on top of the bookshelf, causing child to fall over the bookshelf and the bookshelf flipping over and falling on child, causing a bruise near the eye.

The caregiver did not demonstrate good judgment when seen on video footage lifting a child by the left hand/wrist off the floor before getting a hold of the child?s right hand/wrist, extending both arms, and placing the child in the chair, which resulted in an accidental injury that required medical attention.

Based on video footage obtained, a caregiver was observed forcibly approaching a young child and positioning herself over the child lying on a mat while grabbing the child's face in an attempt to address her behavior during rest time.
It was determined in a DFPS investigation that an incident that occurred on 10/11/2024 where a child was left unsupervised on a playground was not reported in the required 48 hour timeframe.
An incident report for an incident that placed a child at risk was not provided to the parent within 48 hours.
The parent whose child was left unsupervised on the playground was not notified immediately after the incident occurred. The incident occurred on October 11, 2024, and the parent was not notified about the incident until October 23, 2024.
Supervision was compromised when caregivers left a small child unsupervised on the playground. The caregivers who were on a separate playground found the child approximately 15- 30 minutes later and alerted management.
The center director failed to meet her responsibilities and did not inform the parent of a child who had been placed at risk even after she had been notified that an investigation was opened regarding the incident.
Documentation of a fire inspection being completed within the last 12 months was not available for review.
Infant monthly feeding sheets for the month of June have not been signed.
A shelf in the infant classroom was observed to have peeling safety coating. A large hole about 5-12 inches wide and 5-12 inches deep was observed in the school age playground. Exposed piping was observed in the school age playground.
Documentation for drills was not readily available to include Severe weather, fire, smoke, and extinguisher checks were missing for May.
First aid kit in van #1 was missing thermometer and tweezers Van #2 was missing themomerter, scissors, tweezers Van # 3 did not have a first aid kit at all
There was one staff member employed with the operation that did not have cleared background check. Note: The employee was immediately sent home for the day.
