Medication logs for two children did not show what year the medication was administered.

Three caregivers did not complete trauma informed care training in the required timeframe.

Two child plans were not updated within the required 180 days.

Four caregivers did not complete their annual psychotropic medication training in a timely manner.

Incident was not called in to statewide intake within 24 hours.

Multiple background checks for active foster parents were not rerun prior to reopening the homes.

Incident report provided by operation states a child in care was injured on 9/23/2022 while at school. Child was diagnosed with left elbow fracture on 9/25/2022. A report was made to hotline on 9/27/2022.

Foster home study must contain the proper documentation when requested. Foster home study must contain references with the date, method of contact, time conducted, who was present and a detailed summary.

Two of the four homes did not have appropriate documentation for this section.

During the walk-thru the upstairs and downstaris fire extinguishers were found to be expired.

During the walk-thru home medication was found not secured in the master bathroom, master bedroom, and upstairs bathroom.

The child's right was missing from one of the four files reviewed. One of the files reviewed out of four the child's rights was not signed by the caseworker.

Inappropriate jokes were being told in front of the Children in care.

It was found the child in care was not being supervised according to their service plan and safety plan.

There are seven children staying in the home at the time of inspection. Five foster children and two biological grandchildren. Agency case manager had no explanation for two biological grandchildren staying in the home. The foster parents admitted the biological grandchildren have been staying in the home for previous two weeks and will be staying an additional five weeks.

The incident report stated foster parent were aware the child in care tested positive for communicable disease on 01/12/22 and it was not reported to licensing until 01/14/22.

Serious incident report was reported two days after a positive result which is not within the required 24 hour reporting timeframe.

A communicable disease was not reported within 24 hours to Licensing.

A foster parent was diagnosed with a communicable disease on 12/18/21 and it was not reported until 12/21/21.

It was determined a child in care sustained an intentional physical injury.

Household member who resides in the home did not have a completed background check.

A child in care was not being properly supervised which led to unexplained bruising.

Evidence and information gathered support that was a child in care was grabbed in an inappropriate manner causing bruising to arms.
