There was a food substitution on the menu today that was not recorded in an area visible to parents.

There was two infant cribs that were not labled. CI: Director corrected at inspection.

There were two toilet bowls in the school age classroom that had mold buid up. CI: The director cleaned both toilets during inspection.

A spray bottle containing diluted bleach was observed on the children's shelf located in the classroom making it accessible to the children in care. Note: The caregiver removed the spray bottle to a higher shelf.

The medical authorization and treatment forms and emergency contacts were not available in the one van that transports the children in care. Note: This was corrected at inspection as the Director added the required information.

The playground maintenance inspections and repair records were not documented for the previous three months. Note: This was corrected at inspection as the Director conducted and documented the playground inspection for the month of March during the inspection.

A 15 month old was observed being cared for in the same classroom as a four year old. The oldest child in the group cannot be more than 18 months older than the youngest child in the classroom. Note: This was corrected at inspection as the child was moved to the infant classroom.

The electronic safety alarm installed in one of the operation's vehicle used to transport children was observed not functioning during inspection.
