During the course of a DFPS investigation, it was determined that a caregiver failed to ensure that all children in their care were present when transitioning from outdoor play back indoors, resulting in a child being left alone unsupervised on the playground for 30 minutes.

During the course of a DFPS investigation, it was determined that staff failed to ensure that other staff were aware that children were being transitioned to their group. This failure to communicate regarding the transition of children from one group to another resulted in a child being left on the playground unattended for approximately 30 minutes, with nobody noting the child's absence until the parent arrived to pick up the child from care.

There were no provisions listed for moms who are breast feeding their child in the operational policies. This was corrected at inspection when a procedure was written to be added to the operational policies.
The caregiver acknowledged that none of the toddler training cups were labeled. This was corrected at inspection when the assistant began putting labels on the cups.

During investigation, it was determined that the operation withheld food from a child as punishment during a special event in the classroom due to alleged misbehavior.

Observed a 6 month old infant securely sleeping in a restricted device. This was corrected when caregiver removed the infant from the restricted device and placed the infant in the crib.
The caregiver used their foot as a method of intervening to redirect a child. Placing their foot on the side of the child's stomach, causing the child to fall over.
