Director has failed to comply with the required trainings that staff must obtained throughout the year.

All fire extinguishers inside the operation and in transportation vans were last serviced on 11/2020.

3 out of the 6 children's files had incomplete emergency contact information; missing address.

Staff who transport children did not have current trasnportation training.

3 out of 6 children's files do not have updated immunization records.

All staff employed at the operation did not have any current annual training hours.

The caregiver in the infant room did not have current training in SIDS, shaken baby syndrome and early child brain development.

The television on a table in the school aged classroom was not anchored.

Caregiver in the toddler classroom with combined age group of children was observed to leave children unsupervised when opening the front door to children arriving at the operation.

An arrow staple remover was on top of the play kitchen in the 2-4 year old classroom. Correction was made after caregiver removed the tool.

A portable hook up high chair did not have the safety strap in the 2-4 year old classroom. Correct was made when the portable chair was removed.

A 9 month old infant was sleeping in a crib covered with a fleece blanket. Note: Correction was made after Child Care Regulation Inspector informed caregiver to remove the blanket from the infant immediately.
