Based on information collected for this investigation, it was determined a child was left unsupervised on the playground.
The six passenger stroller had three safety straps in poor conditions.
The rugs in the school age, Pre-K and toddler classrooms were dirty and require a deep clean. The floors in the school age, Pre-K, and toddler classroom were found to have debris in the immediate play area and around the shelves and corners of the rooms. The floors were sticky. There is a shelf between the door to the toddler classroom and school age classroom that was broken. There is a bouncy in the infant room that was broken. There was a purse on top of a bouncy in the infant classroom. There is a foul smell in the kitchen, school age classroom, and trash can outdoors.
During the monitoring inspection, it was learned that staff are on their cell phones throughout the day.
There was a picket on the playground fence that was broken and missing a piece. There were other pickets slightly curved and bent.
The door to enter and exit the Pre-K classroom is jammed and does not easily open.
During an investigation, it has been found that the operation has kept and given out expired milk/food items to children in care.
During the investigation it was determined that the director did not comply with the responsibilities of ensuring the day care operated in compliance with Minimum Standards regarding the safety of food/beverage items fed to children in care.
Both binders containing the children's information did not include any signed medical authorizations for each child that is transported. NOTE: This was corrected during inspection when copies of all medical authorization forms were made for each child that is transported.
Van #1--Two of the nine booster seats in a transport vehicle were expired. NOTE: This was corrected at inspection when both booster seats were removed and replaced with non expired booster seats. Van#2--Five of six booster seats were expired. NOTE: This was corrected as the expired booster seats were removed and replaced with new booster seats just purchased.
During the investigation, it was determined that there was an individual at the operation without a background check. The director's sixteen year old daughter had been to the operation several times to conduct office duties.
A 5 month old infant was observed wearing a bib while sleeping in crib. NOTE: This was corrected when the bib was removed.
During inspection two children were observed coming out from restroom and not washing their hands before going back to playing in the classroom.
The restroom in the Pre K classroom did not contain any toilet paper.
During the investigation; it was determined that the caregiver/driver did not conduct the required steps in ensuring there were no children left behind in the transportation vehicle.
It was determined during the course of a DFPS investigation that neglectful supervision occurred when a child was left unattended in the transportation van for over an hour. The operation was unaware the child was not with the group until the child's mother arrived to pick them up from care.
Operation staff reported during a DFPS investigation that the child safety alarm in the transportation vehicle was not operational.
One of eleven employee files did not have any age/education qualification included in the personnel file.
Two of the three drivers do not have verification of having completed the required 2 hours of transportation training.
At follow up, the files were not put together/complete for all staff.
The director did not have a current copy of her CPR/First Aid card.
One of six children's files did not have an updated shot record.
The office manager was not available, the director was unable to get into locked cabinet.
In the first aid kit located in van, there were no gloves to make kit complete. The director corrected the issue when she added gloves to the kit.
The Pre-K classroom did not have an activity plan posted.
Upon entering infant room, it was observed the ceiling remained unrepaired. Note-infant room remains closed due to damage.
The fire extinguishers in both transportation vans were last serviced in Sept/2020.
The Toddler classroom has exceeded their one hour of screen time. The televsion has been on for a hour and 30 mintues.
In the afterschool classroom there were cleaning supplies that were left on a shelf that were accessible to children. Also in the afterschool bathroom there was hydrogen peroxide that was left underneath the sink without a lock on it. Note: This was corrected at inspection when the director removed the cleaning supplies from the classroom and the bathroom.
In the infant room there is a hole in the ceiling due to water damage. Note: The infant room is closed down due to the open ceiling, until it is fixed.
There were two air filters, one in the Preschool room and the other in the Infant room, that were observed to be a dark gray color almost black. There was also a hole in the ceiling due to water damage. There is still water slowly dripping from the hole located in the infant room. There was also debris from the ceiling tiles falling out located on the floor in the infant room. Along with the debris was a cockroach. Note: The operation changes the air filters every month. The operation has pest control come out every month as well. The infant room is currently not being used.
