Two classrooms did not have emergency evacuation diagrams posted.

The climbing playground structure in the three's outdoor area does not have the appropriate surfacing installed underneath it.

A deficiency was cited when the operation could not provide a signed incident report.

There were cleaning supplies located underneath the sink, in an unlocked cabinet, in the Pre-K bathroom. Note: This was corrected at inspection when the director removed the cleaning supplies and put them up on a high shelf, where children can not access to the cleaning supplies.

The swings on the after-school playground had big screws that were protruding out of the pole that the swings were attached to. Additionally, the swing seats were cracked, and their chains were twisted.

There were two vans meant to transport children that did not have complete first Aid kits available for review. One van was missing band aids, and the other van was missing band aids, and tweezers. Note: This was corrected at inspection when the director put band aids in the first van's first aid kit, and put band aids and tweezers in the second van's first aid kit.

There were two children files out of the twelve that were viewed that did not have a health statement signed by a health care professional, available for review.

There was one child file out of the twelve files viewed that did not have an updated immunization record available for review. Note: This was corrected at inspection when an updated immunization record was placed in the child's file.

One caregiver in the infant room failed to wash the child's hands after changing the diaper.

There was no current annual fire inspection report available during inspection.

In the school age classroom there was one television found to be sitting on a table, The television was not achored or secured. The television was accessible to children. The television was removed and put into storage during insepction.
