Video footage showed the 2 year old caregiver stepping out of the classroom leaving napping children unattended. Video footage of the infant room shows an infant caregiver asleep while feeding a child leaving the children unsupervised.

Video footage obtained shows a caregiver scrolling on her phone while supervising a group of napping children.

Video footage reviewed shows an infant was asleep in a swing for approximately 8 minutes.

The director was found supervising both the preK class of 6 children and the toddler class of 18 children with SAG of 2 years totaling 24 children to one caregiver. This was corrected during inspection as staff reported for their shifts within 20 minutes, three caregivers arrived for PreK, and two toddler classrooms.

The infant classroom was found over ratio by 2 infants. Caregiver was caring for 6 infants with specified age group of 10 months. The child:caregiver ratio for this class is 1:4. The director was found supervising both the preK class of 6 children and the toddler class of 18 children with SAG of 2 years totaling 24 children to one caregiver. This was corrected during inspection as staff reported for their shifts within 20 minutes and the director was able to join the caregiver in the infant classroom. Three caregivers arrived for PreK, and two toddler classrooms.

The operation has not removed the previous director from the controlling list. The previous director was inactivated in Oct/2022.

Three large toy trucks were observed to be broken with jagged edges. This was corrected at inspection when a caregiver removed all three from the playground area.

746.603(a)(3) - Children's Records - Health Statement

The under-sink cabinets in the restrooms contained lysol and cleaning products that are accessible to children. There are no security latches for the cabinet doors. This was corrected when all supplies were taken out and placed in a locked cabinet/storage.

746.605(12) - Required Admission Information - Emergency Medical Authorization

The operation did not have a documented record of monthly smoke detector/carbon monoxide tests conducted.

An infant was observed sitting in a bouncer seat for longer than the allowed time frame of 15 minutes. NOTE: This was corrected at inspection when infant was pulled out of bouncer and placed on floor to continue.

Three staff files were missing a copy of the driver's license/identification card, proof of education, background eligibility letter and annual training. One staff file was missing proof of education and annual training.

A child was sitting in a booster seat without being secured with the safety strap. This was observed twice when the child was moved to a high chair in the toddler room when doing an art activity.

Children were running in the classroom. There were large building blocks all over the floor. Some children were screaming and crying.

The infant room was observed to be missing an adult sized chair/rocker.

3 infants were observed sleeping in unlabeled or mislabeled cribs.

Cups were observed to be missing labels of the child's first name and last initial. Note: This was corrected at inspection when the caregivers wrote the names on the cups.

Daily reports were not available for review and infant caregivers admitted to not completing any daily reports (written nor electronic) for parents.

A review of the Employees Who Left Employment in 2020 Survey data from March 18, 2021, revealed that the center had not reported the number of employees who left employment in 2020 by January 15, 2021, as required.
