Six out of ten staff files evaluated were missing CPR and First aid training. Note: Operation had a schedule training for all staff on the following day.

A staff member's purse in the five-year-old classroom was sitting on the children's low open shelve accessible to the children in care. Note: This was corrected at the inspection.

A caregiver had documented inappropriate discipline measures as a child colored the floor. The caregiver made a report stating that the child will clean the floor first thing the next morning. It was documented that the child was put to clean floors at arrival time.

A caregiver was found prohibiting a child to play with toys or in centers due to the child's daily behavior.

There was observed to be less then 6 inches of loose-fill surfacing material located in the areas of the slide exits on the Toddler and Pre-K playgrounds.
