Two of the six staff files did not have 2021-2022 annual training for the following; Recognizing and preventing shaken baby syndrome and abusive head trauma. Understanding and using safe sleep practices and preventing sudden infant death syndrome (SIDS) and Understanding early childhood brain development.

Two of the six children files didn't have a current immunization record on file.

No documentation of the smoke detector, fire extinguisher or carbon monoxide was provided at the time of inspection. NOTE: This was corrected the inspection.

Toddler playground boarders was observed not to be secured as the screw was missing and protruding sticking up out of the black plastic boarders posing a tripping hazard. In addition to the bike ramp that was observed lifted posing a tripping hazard.

The following trainings were not available in the files for the six staff files: Emergency Preparedness Communicable diseases Food allergies Understanding building and premises and Handling, storing, and disposing of hazard material Four of the six staff did not have Abuse and Neglect training available.

One of the six children files is missing Authorization information. Two of the six children files were missing admission dates. (Corrected at inspection) Five of the six children did not have special care information. One child with no food allergy information. Three of the children files did not have either the address or phone number for emergency contacts.

In the PreK classroom cabinet under the sink a bottle of Bleach was found to be accessible to the children. No childproof locks was observed. NOTE: This was corrected as the bottle was removed to a cabinet with a lock and key.

The PreK classroom was observed eating on the floor while watching a movie. NOTE: This was corrected when the children were moved to the table to finish eating.

Documentation of the building grounds checklist was available for review. NOTE: Corrected at inspection when the building grounds checklist was conducted.

Children files were not available for review.

Both fire extinguishers in the building were last served on February 2019.

The emergency preparedness plan did not specify procedures for; Communicating with authorities, Relocation diagram, Essential documents Continue care, and Reunification.

The PreK classroom was observed eating a snack while watching a movie. NOTE: This was corrected when the television was turned off.

A current fire inspection was not available for review.
