During the inspection, debris was observed in an infant diaper changing station. Additionally, classrooms were found to have debris on the floors and a Pre-K classroom restroom was also noted to have debris stains on the walls.

It is found that a child with known special needs was not supervised to the level of care that they are known to need, which allowed them to exit the operation unsupervised and proceed to the edge of the parking lot of the operation. The child was immediately returned to care unharmed after being stopped by the parent of another child.
