In the pre-k classroom there was water conditioner liquid and cleaning supplies that were accessible to children.

It was determined in the investigation process that an individual with a volunteer background check clearance, was left unsupervised with a group of children.

An incident report was not written for an incident in which a child was able to leave the operation for a minimum of 20 minutes.

Supervision was compromised when a child was able to leave the operation's premises unnoticed.

Neglectful supervision occurred when a child was able get 3/4 of a mile away from the operation unnoticed for a minimum of 20 minutes. The child was found by a passerby unharmed.

Through the investigation process it was determined that the person left in charge, in the director's absence, was not qualified as she did not have a current and clear background check.

During diapering, a caregiver was observed to not wash the child's hands.

A caregiver was observed utilizing the handwashing sink to rinse bottles. Noted: This was corrected during inspection when the caregiver was given information on why the handwashing sink cannot be used for cleaning bottles.

During diapering, a caregiver was observed to not change gloves before initiating a diaper change of another child. Noted: This was corrected during inspection when the caregiver removed her gloves and washed her hands before the next change.

Caregiver stated that children age 12m- 2 years, who are all walking, are sleeping in cribs.

The cribs were observed to not have labels or a system to identify the crib to the child that uses that crib.

A current Fire inspection was unable to be provided for review.

A child was left alone in a classroom, when the caregiver left the room to open the door to allow inspectors in to the building.

The classrooms being utilized were changed to accommodate different age groups before notifying licensing.

Activity plans for the month were not posted.

Three of the eight children's files viewed did not include the required phone number or address for the emergency contact.

Two of the eight files observed did not include a health care provider statement, past the 12 months after the enrollment date.

Spray bottles were observed to be on the changing table accessible to children. In the children's bathroom, a cabinet was not utilizing the installed safety locks, leaving the cleaning supplies stored in cabinet to be accessible to children. Noted: this was made complaint during inspection when the cabinet was locked and the spray bottles were moved to a higher cabinet.

There were items in the first aid kit that were missing. The items that were missing were: Thermometer Gloves

There were two caregivers at the operation that did not have their CPR and First Aid certification available for review.

The television in the Preschool room is not anchored to the shelf that it is set up. The operation corrected this at inspection when the television was removed form the classroom.
