Abuse occurred when a caregiver intentionally bit a child on the arm with enough force to cause a distinct bite mark which resulted in a bruise that remained for several days.

An incident report was not completed after a child was placed at risk.

One child had listed food allergies on the enrollment form and did not have a food allergy plan on file. This is a recite.

10 out of 10 staff files were missing Medication Administration Training.

6 out of 10 child files reviewed did not contain current health statements from the child's physician after being enrolled for 12 months. This is a recite.

1 out of 10 staff files was missing 15 staff training hours during their annual evaluation period.

The operation was currently updating all staff files and training certificates. The inspector was not able to evaluate all required employee paperwork, start dates, and verify training hours for 10 staff files evaluated.

3 out of 10 child files reviewed were missing allergy information on the children. One file, the parent did not mark any known allergies on the enrollment form, One child has a listed peanut allergy and use of an epipen but did not have an allergy plan on file. One child had listed food allergies on the enrollment form and did not have a food allergy plan on file.
