A combined classroom was with eleven children had a eighteen month difference between the youngest child and oldest child. The age of the youngest was twenty months and the age of the oldest was five years old.

Plastic window in the playscape was broken and opening was big enough where children put the hand through. The broken part of the window had sharp edges that could have potentionally harm a child. This was corrected at inspeciton when director removed the broken plastic window from the playscape.
