A hired maintenance member with access to children was present at the operation without background check status of Eligible, Eligible with Conditions, or Provisionally Eligible with Conditions. Deignee was advised to run the backgroundcheck as soon as possible.

The infant feeding instructions for four infants were not updated within the last 30 days. The last time they were updated was on March 1, 2022.

Based on interviews with staff and observation caregivers were not actively present in the preschool classroom during naptime.

Based on interviews and observation of the preschool classroom during naptime, children were being left unsupervised for two to three minutes when caregiver would leave the classroom to open the front door or take a child to the restroom.

Based on sign in and out logs and talking with staff director has not been routinely present in the two last weeks.

Director had not completed the 30 hours of annual training.

Caregiver was not current for her SIDS/Shaken baby training

There was sufficient evidence to determine the neglectful supervision of a ten-year-old child who left the operation unattended and walked home, approximately 15 minutes away. The caregivers were unaware of his absence until the parent called the facility to notify them.

The director did not ensure that staff were aware of their responsibilities or that each caregiver was assigned a lunch period that assured appropriate supervision coverage. As a result, a child left the operation unattended.

Supervision was compromised when two caregivers went to lunch at the same time leaving some of the classrooms with napping children unattended.

There was sufficient evidence to determine a caregiver was leaving some of the classrooms with napping children unattended.
