There were six infants ages two months to thirteen months with one caregiver. The maximum amount of infants for this age group is four infants per one caregiver, therefore this group was not compliant for proper ratio. This was corrected at inspection when another caregiver arrived to bring the ratio into compliance.
Four out of five bottles in the infant room were not labeled by name as required. This was corrected at inspection when the names were added to the bottles.
Three out of eight children's records evaluated were missing the required emergency contact information.
The operation did not have the required Child Product Safety Commission Form. This was corrected at inspection when the director downloaded the form, signed it, and posted it.
During naptime, it was observed that a four month old infant was combined in the same room with older children up to four years in age. Minimum standards state that children under eighteen months of age are only allowed to be combined with older children as long as the oldest child is less than eighteen months older than the youngest child, therefore the operation was not in compliance. This was corrected when another caregiver arrived, and the infants were separated from the older children.
Based on information gathered during the course of the investigation, caregiver did not give child his medication at 10:30 am as indicated on the medication log.
