A wasp's nest with a wasp in it was seen on the playground under the roof of a playhouse. A nail was protruding out of a playhouse accessible to children located on the infant playground. A broken toy with sharp edges was accessible to children. NOTE: These 3 hazards were corrected at inspection when the assistant director called Pest control and scheduled for them to come out on today. The assistant director was able to screw the nail in from protruding out of the playhouse roof area. The assistant director removed the broken toy from the playground.

Recited: Temperature measurements taken in three classrooms during the follow up inspection were lower than the city mandated lowest temperature allowed for child care facilities.

The permit holder has failed to adequately respond to work order requests submitted by the operation regarding the HVAC system not working properly. This failure to ensure that all classrooms in the building are kept at a consistent temperature has resulted in some classrooms being too cold in the winter and/or too hot in the summer for children in care.
Based on evidence collected during this investigation, it was determined that the operation failed to ensure that the infant classroom was warm enough for infants in care. The classroom was observed to be very cold, with temperatures measured as low as 64.6⁰ F. Other classrooms measured within a degree of two of this temperature, and one classroom that was not in use at the time measured at 56.5⁰ F.
A caregiver was present at operation without an active background check. NOTE: Caregiver was sent home and told must not return until results are returned.
A caregiver was observed grabbing a child's forearm to stop them from grabbing plastic gloves used for serving snack and yanking them from their hand roughly.
A classroom for two year old children was observed with 12 children with one caregiver. The SAG in the classroom was two years. Note: this was corrected at inspection when a three year old child was bumped up to a preschool classroom.

One of ten caregivers did not have verification of up-to-date CPR/FA certification.
A caregiver in the toddler classroom gave the incorrect number of children in class during naptime. Caregiver pulled the roster on the tablet and compared with the written list and found the error.
During an investigation, it was determined that a caregiver was using harsh methods of waking children from nap by lifting the cots there were laying on with her foot.
A caregiver changed an infant's diaper and released them without washing their hands when done. A caregiver did not ensure a toddler's hands were washed after changing their diaper.
The mulch was low at exits of slides and high activity areas. The director did not know when it was last spread/raked.
Each fire extinguishers in each of the two transportation vehicles did not indicate a service date in the tag.
The operation was 83 degrees at the time of the inspection due to AC malfunctioning since Tuesday 6/20.
A caregiver was observed using aggressive tones with the children in her care.
A caregiver in the school age classroom was observed redirecting a child in a loud voice. The child was visibly upset during the occurrence.
There was no incident report created within the adequate timeframe and the parent was not given a written notification on the details of the incident.
During the investigation, it was determined that a caregiver mistreated a child by pinching the child while in naptime causing a lesion.
The operation is impeding an investigation by refusing to release video footage taken the day of an alleged incident that is related to an open investigation.
Two of ten children's files did not have an immunization record in file. One of ten children's files did not have a signed health care statement from their physician. Two of ten children's files did not have the child's school information.
The fire inspection was overdue as of 4/27/23.

During an investigation, it was determined that a caregiver placed a crying child roughly outside the classroom door and closed the door to the classroom leaving child alone in another classroom.
A child in the Pre K classroom did not wash their hands after having diaper changed. Note: this was corrected when caregiver was told to ensure child washed their hands.
While reviewing children's files, 2 of 6 records reviewed did not have immunization records or statement from parent saying they refuse to immunize in the child's record.
While reviewing annual training records: 4 of 19 employees did not have training records available for review. Four caregivers had not completed their required 24 hours for the 12 month period. The director had only 18 of 30 hours completed for her annual training.
During investigation, an employee (YG) was observed working as caregiver while background check remains pending. Director stated she has been working since Wednesday, 5/25/22. Employee was told to go home and must not return until background results are returned.
Three tenured employees of 19 employees total, did not have current CPR/First aid certifications.
During investigation, it has been determined that yelling does occur at operation from caregivers towards children.
During investigation it was determined parents were not immediately notified of child's injury. Once an attempt was made to notify one parent, no additional attempts were made to contact second parent or anyone else when contact not achieved.
