Lunch had been prepared and left cooling on stove uncovered for over an hour; it was then served to children during lunchtime. An unopened gallon of milk was found in refrigerator with the expiration date of 11/29/23. This was corrected during inspection as the milk was thrown out.

The restrooms had toilet brushes and a plunger accessible to small children. This was corrected during inspection when the items were moved and made inaccessible to children.
Three of the four fire extinguishers in the operation had not been serviced in over one year. Two had last service date of May/2022 and the third has been serviced in November/2022.

A dead rodent was found on the infant playground
The inside of the slide had was broken and had hole covered by tape.
Minimal rodent feces was found in three classrooms.
Two out three infants were not securely strapped in the high chairs while they were eating.
The loose-fill on the playground did not meet the proper depth, and grass was seen growing over the loose-fill.
The toys and furniture in the infant and two-year-old classroom were soiled and had not been properly cleaned. A rug in the school-age classroom had been chewed up in multiple places by rodents. The outside infant toys were soiled with dirt, torn, and had standing water.
Rodent feces was found in multiple classrooms, including the school-age and two-year-old classrooms. Rodent feces were also found in the kitchen closet. The school-age restroom had not been adequately cleaned and had a strong order of urine.
Mouse traps and an open trash bin were observed in the school-age classroom assessable to children.

Staff failed to remove the chemical cleaner left on surfaces within the operation resulting in four children becoming exposed to the cleaner and needing medical attention.
Operation failed to contract emergency services when four children came into contact with a chemical cleaner, which caused a severe reaction.
The director failed to ensure the staff were adequately trained in their daily duties and failed to respond appropriately to children becoming injured in care.

During the course of the investigation it was proven the operation has infestation of Rats. Rat Exerment was seen in classrooms staff confirmed seeing rats during operating hours.
Staff did not notfy parents in regards a communicable disease in the infant classroom.
The infant feeding schedules were not up to date for the current month.
Operation did not notify the parent about the incident until they arrived at the operation.
