The school age children and infant classrooms were missing activity plans. The activity plan in the two-year-old classroom was incomplete missing dates, times, and some activities.
A yellow bus playground structure was not properly anchored. Four bolts were exposed above the ground. The bolts present a high tripping hazard to children. Photos were taken and uploaded to digital storage.
An under layer plastic cover was observed exposed in proximity of a playground bicycle merry-go-round. Children running around near the playground structure are at risk of tripping and falling. A permanent plastic cover on a cemented surface where children play was also noted as a possible tripping hazard. Photos were taken and uploaded to digital storage.
The sinks in the infant room and the toddler room designated only for hand washing were been utilized to wash bottles and other children's items. Corrected during inspection all items not related to hand washing only were removed from the sinks. Photos taken and uploaded to digital storage.
The use zones for several play structures were less than 6 ft apart.
The operation did not have an emeregncy diagram posted in the classrooms or by exits.
The operation had tree branch growing within the playground area that can cause a potential tripping hazards for the children. Several of the play structures had spider webs or damages pieces. The outside wall is portuding out.
