There was a piece of climbing equipment that did not have 6 inches of loose fill surrounding the structure.
There was a tunnel that the children can climb through that was broken on top of the tunnel on the pre-K through School Age playground. Note: The director quickly removed the tunnel out of the outdoor play space.

It was determined that a caregiver had given a child their cell phone to watch videos to calm the child down during naptime.
It was determined that a caregiver did not present good judgement when a hot tray was placed on a shelf accessible to children, as a result the child was burned.
It was determined that a caregiver was telling a child to kick themselves instead of kicking others in the classroom.
During the course of the investigation, it was determine that a school-age child around 8 years of age is allowed to help in the infant room making the oldest child in care more than 18 months older that the youngest child in the infant room.
The door to the indoor gym had a hole at the bottom of it. Weed prevention tarp on the toddler playground had come up through the loose-fill creating a tripping hazard. The fence on the school age playground is separating from the building creating an approximately 11 inch gap which can become an entrapment hazard.
During the course of the investigation, it was determined that children are only provided one snack in the afternoon. Children that stay later are not provided an additional snack.
The operation's annual fire inspection was not completed timely. The last fire inspection was conducted on 06/11/2020. Note: The operation applied for their annual fire inspection on 11/16/2021.
The gate that leads to the front of the building from the playground were locked and keys were not easily accessible for caregivers in the event of an emergency evacuation.
