An individual with a provisional background check was obsered to be alone with children.

There were five children unsupervised in a room.

During the investigation process, it was determined that infants (0-17 months) were being combined with children greater than 18 months older than them.

During the investigation process it was determined that after being made aware that a child sought medical attention for an incident that occurred while the child was in care, that a report was not made to Child Care Regulation (CCR).

During the investigation process, it was determined that children, of all ages, were being grouped together for two hours at a time from 6:00-8:00 AM and being grouped again in the afternoon from 5:30-6:30 PM.

The operation did not have documented tests of the smoke detectors in 2023 and January 2024. Noted: This was corrected during inspection when the smoke detectors were tested.

Fire extinguishers should be inspected by a designated staff member frequently, but minimally once a month. These inspections should be documented. Noted: This was corrected during inspection when the fire extinguishers were inspected.

There was not a current employee list posted. Noted: This was correct during inspection when a current employee list was created and posted.

Carbon monoxide detectors were not tested in the year of 2023 and in January 2024. Noted: this was corrected during inspection when the carbon monoxide detectors were tested and documented.

The operation did not have a documented fire drill for the months of November and December 2023. The operation also only had one Shelter in place drill docuemnted for 2023. Noted: Moving forward the operation will make sure that all drills are documented.

The operation did not have an emergency prepardness plan for review.

One Staff file did not include the necessary employee affidavit.

Three of the ten children's files did not include updated immunization records.

Five of the ten Children's records reviewed did not include all necessary information for emergency contacts listed.

An infant was observed to be sleeping in a restrictive device while also being wrapped in a blanket. Noted: This was corrected as inspection when the child was removed and placed in a play yard.

The carpet in the infant space was observed to have mold.

Children in care are being given screen time as an activity for longer than one hour per day.

Children are being placed in a restrictive device (high chair) to discipline the child.

There was one child file out of the ten that were viewed that did not have the name, address, and phone number of the child's health care physician.

There was one child file out of the ten children files viewed that did not have an updated immunization record on file.

A lagre black trash bag full of garbage was observed in the school age outdoor playground while the children were playing.

It was determined that in December a caregiver forcefully pushed a child to the ground.

There was expired hydrogen peroxide in the first aid kits located in the center and on the transportation vehicle. The expiration date stated November 2020. Note: This was corrected at inspection when the director bought some new hydrogen peroxide and placed them in the two first aid kits.

There was a piece of climbing equipment that did not have 6 inches of loose fill surrounding the structure.

There was a tunnel that the children can climb through that was broken on top of the tunnel on the pre-K through School Age playground. Note: The director quickly removed the tunnel out of the outdoor play space.

The infant caregiver was observed using her phone while supervising a group of infants.

A three month old infant was observed in a bouncer seat without being strapped in.

A three month old infant was observed sleeping in a bouncer seat. According to the caregiver, the infant had been asleep for 30 minutes.

The floor in the 3 year old classroom was visibly dirty with several area of dirt around the changing table and sink.

It was determined that a caregiver had given a child their cell phone to watch videos to calm the child down during naptime.

It was determined that a caregiver did not present good judgement when a hot tray was placed on a shelf accessible to children, as a result the child was burned.

It was determined that a caregiver was telling a child to kick themselves instead of kicking others in the classroom.

During the course of the investigation, it was determine that a school-age child around 8 years of age is allowed to help in the infant room making the oldest child in care more than 18 months older that the youngest child in the infant room.

The door to the indoor gym had a hole at the bottom of it. Weed prevention tarp on the toddler playground had come up through the loose-fill creating a tripping hazard. The fence on the school age playground is separating from the building creating an approximately 11 inch gap which can become an entrapment hazard.

During the course of the investigation, it was determined that children are only provided one snack in the afternoon. Children that stay later are not provided an additional snack.

The operation's annual fire inspection was not completed timely. The last fire inspection was conducted on 06/11/2020. Note: The operation applied for their annual fire inspection on 11/16/2021.

The gate that leads to the front of the building from the playground were locked and keys were not easily accessible for caregivers in the event of an emergency evacuation.

I observed 3 out of 10 children's records were missing immunization records.

A background check for two people no longer employed with the operation was not inactivated within seven days. Note: This was corrected at inspection as the background check was inactivated during inspection.
