Based on the information gathered the 12 to 18 month old children were placed in time out for more than two minutes per year of age.

Based on the information gathered one caregiver was grabbing the children roughly and aggressively.

A child in the 13-18 months classroom and a child in the 18-24 months old classroom were found using pacifier beads. Note: Director removed beads immediately from the children.

6 out of 10 staff files did not have the required annual training pertaining to: Building/premises safety, responding to food allergy emergencies, administering medication, hazardous materials, child abuse and neglect.

Two out of ten children files were missing updated immunizations. Note: One immunization record was corrected at inspection when the parent's sent a current record via email.

During the course of the investigation, it was determined that parents were not immediately notified of a situation that placed their child at risk. The parent was notified an hour later by the operation regarding the incident.

A child was left on the playground unsupervised after the class transitioned back into the classroom. The child was found after an hour and forty minutes later. When found, he was red faced, sweaty and crying.

During the course of the investigation it was determined that 2 caregivers neglectfully supervised a three year old child when that child was left on the playground for an hour and forty minutes.
