A first aid kit was missing tape and a thermometer.

Two of ten children's files did not have a health care provider statement.

One of ten children's files did not have a current immunization record.

Three of ten children's folders did not have an address listed for the emergency contact.

A child with known food allergies did not have an action plan on file.

A caregiver was observed failing to wash their hands after diapering a child.

Video shows a caregiver failing to wash the hands of a child they diapered.

Video shows that the safety straps were not used for a child who was placed in a highchair.

Video shows a caregiver not appropriately supervising an infant in a highchair when the caregiver is observed sitting down in a blind spot and not keeping the child in view.

A caregiver did not demonstrate good judgment when they failed to use the required safety straps to buckle an infant into a highchair.

Bucket swings in the toddler playground were found not having the 6 feet of use zone on the side of the adjacent swing and the fence. *Swing was removed immediately.

Nine bottles, in the Lambs Classroom were not labeled with the child's first name and last initial. *Note all bottles were labeled at the time of the inspection.

Based on interviews the operation was using threats for discipling purposes. Children were being told that if they did not behave they would be taken to a specific caregiver.

The two playgrounds (12-18 months and 2 year old) in the operation were noted to have gravel covering concrete.

The operation did not provide an incident report for an injury that required medical attention.

Three out of three transportation vehicles fire extinguishers were not securely fastened. It was noted that the fire extinguishers were loose in the vehicles floors.

There were 10 employee files that were viewed, those 10 employees did not complete training on Preventing and responding to emergencies due to food or an allergic reactions.

There was one child file out of the ten that were viewed, that did not have an updated immunization record available for review.

There was 4 infant feeding schedules out of the 5 that were viewed, that were not dated and signed by the infant's parents.

There was observed to be less then 6 inches of loose-fill surfacing material located under the bucket swings on the toddler playground, and the swings and slides on the 3 years and up playground.

There were 2 caregiver files out of the10 that were viewed, that did not have the required 24 hours of annual training. One caregiver was missing 11 hours of annual training and another caregiver was missing 6 hours of annual training.

There was an infant's bottles that were not labeled with the child's first name and last initial.
