Crackers were served on a bare table for children to eat from.

The following personnel records were not available for review at time of inspection: proof of education, notarized Affidavit for Applicants for Employment form, proof of background check eligibility letter, copy of photo ID, and signed statement employees recieved copies of operational policies and employee policies.

A piece of outdoor play equipment was observed in front of a slide on the playground. The equipment overlapped the slide's required use zone.

A vehicle that is used to transport children is not equipped with an electronic child safety alarm.

Based off information gathered, a child was bitten by another child when the classroom was out of ratio by one child. There were 6 infants present with a specified age group of 15 months with only one caregiver.

Based off information gathered, a parent wasn't notified at pick up the same day their child was bitten by another child. The parent was notified two days later.

Five of ten children's files did not contain up to date immunization records.

Three of twenty two caregivers did not have a current CPR/First Aid certificate on file.

During inspection it was observed that one caregiver had 8 children in care ages 8 month, 9 month, 9 month, 9 month, 10 month, 10 month, 12 month, and 18 month. Based on the ages of the children caregiver is out of ratio.

Caregiver's used poor judgment when they failed to touch the playground equipment that is unshaded, ensuring the playground equipment was cool enough for children to play. This resulted in a child receiving a burn on abdomen area.

Two fire extinguishers in the center vans have not been serviced, as required.

The operation did not have a current fire inspection. The last fire inspection was conducted on 8/3/2020.

Based on information gathered during the course of the investigation, it was determined that a 2 year old child was left unsupervised for 1 minute on the playground.

Based on information gathered, the center did not make a report to CCR, that a 2 year old child was left unsupervised for 1 minute on the playground.

Based on information gathered, parent was not informed that child was left unsupervised for 1 minute on the playground.

Based on information gathered during the course of investigation it was determined that two caregiver's placed a child in the restroom for time out with the lights off and door closed. During the course of the investigation it was determined that caregiver was handling the children in a rough manner.

Based on information gathered, the center failed to report inappropriate discipline that occurred when a child was placed in restroom for time out, with the lights off for an undetermined amount of time.
