Based on the investigation conducted, it was determined that a blanket was placed over a child's head during naptime.
The operations diapering chnaging surfaces was above floor level without proper saftey mechanism in place. This was corrected at inspection when the operation begin using a mat to change children on the floor. The director also purchased a saftey rail to be installed on the counter surfaces.
