An infant was moved directly to the group feeding table after having their diaper changed without having their hands washed. This was corrected at inspection when the caregiver was advised to remove the child and wash their hands.

Evidence gathered for this investigation determined the daily report for an infant had incorrect information on the third feeding on time and ounces served to the infant.
