It is found through investigation evidence that a child was left unattended in a classroom for 2 minutes.

It is determined though an investigation by CCR that 2 children were left unsupervised on the playground while in care.

This was found through an investigation that a child was alone, unsupervised in a hallway for 1 minute and 15 seconds.

While reviewing surveillance footage it was found that a caregiver did not wash their hands after changing a child's diaper.

While reviewing surveillance footage it was found that a caregiver used a disposable wipe to clean a child during a diaper change and then used the same wipe on the child's face and hands.

While reviewing surveillance footage it was found that a caregiver did not wash an infant's' hands after changing their diaper.

The fire inspection was over due as of 2/17/23. The fire alarm inspection was done on 1/18/23. The fire extinguishers were inspected 2/6/23.

During the investigation, it was determined that the operation failed to notify CCR of a child injury that received medical treatment within the required timeframe.

There were two staff members that have been present and working in the presence of children at the operation with pending background checks.

There was an Epi Pen on file for a child that is currently enrolled, that had expired in July 2021.

A caregiver in the Pinginos classroom (2-3 year old) was observed changing a diaper without washing hands and changing gloves in-between another diaper change. NOTE: This was corrected at inspection when the director addressed the caregiver as hands were then washed.
