There is a mouse trap in the toddler classroom located behind an unlocked door.

The playground has several toys that are dirty, and need to be repaired. There is also fungus growing in the mulch, and a gap in the gate of the fence that leads to the outside of the center.

During the course of an investigation conducted by DFPS, it was determined that child was neglectfully supervised by a caregiver. The child exited the facility unnoticed and was found by a passerby across the street. The child was gone for approximately 3 minutes and found unharmed before being brought back to the operation.

It was found during a DFPS investigation that there were no additional caregivers available in the building during naptime.

It was found that an identified parent was denied pickup of their children.

There were nine children with one caregiver with a specified age group of 12 to 17 months. This was corrected at inspection when four children were moved to the other infant rooms.

Caregivers failed to use good Judgment when they felt it was appropriate to speak to children in a disrespectful manner. Note: caregivers are no longer employed by the operation.

The loose fill on the playgrounds have not been maintained since last inspection. Loose fill did not measured at the proper depth.

Two children did not wash their hands right after having a diaper change. Their hands were washed after reminding the caregiver.

Nap cots in some of the classrooms were not labeled or numbered using a nap chart.

3 out of the 7 personnel files evaluated were missing annual training hours from the last full year of employement.

A child with a diagnosed food allergy does not have a food allergy emergency plan signed by a health care professional on file.
