A review of the Employees Who Left Employment in 2021 data from February 23, 2022, revealed that the center had not report the number of employees who left employment in 2021 by February 22, 2022, as required.

2 out of the 12 children's records observed are missing emergency contact information for someone other than parents.

U-zone on both playgrounds are lacking required amount of loose fill. Exposing the dirt. 2 and younger playground playscape slide has loose-fill measuring at 1/2 inch. 3 and older playground playscape has 2 slides with loose-fill measuring at 2 cm each.

2 out of the 12 children's records observed are missing immunizations.

7 out of 12 children's records observed are missing admission dates.

2 out of the 12 children's records observed are missing parent's checking off 12 month timeframe for physician signature that have been enrolled under a year.

Water hose was loose covering the sidewalk on the 2nd playground. Numerous pieces of trash was observed on the 2nd playground. Bunch of trash observed packed along the fence on the outside of the 2nd playground. Water hose was corrected when caregivers immediately rolled up hose.

1 out of the 12 children's records observed are missing physican signature in Admission Packet.

Observed a two year old climbing and sitting on top of the toy shelve, while another two year old stood up in an chair in order to climb on top of the same toy shelve, standing behind the other child. This was corrected when caregiver removed both of the children from the toy shelve.

Observed a infant sitting in the high chair drinking out of cup, safety strap were dangling pass the infant's feet. This was corrected when caregiver removed highchair cover and securely fastened the infant in the highchair.

2 out of the 6 classrooms observed, caregivers provided incorrect count for the children they were responsible for, after numerous attempts to collect totals.

Observed trash (plastic cups, paper towels, etc) on the toddler playground (side of building) and the big kid playground (behind buidling). Observed food pieces and crumbs on the floor (from breakfast) and rug while the caregivers and children were sitting on the rug having circle time.

Observed an infant on top of the table while caregiver had her back towards child and working on white board. This was corrected when this was pointed out to caregiver and she removed the infant from the table.

Observed food crumbs all on the rug and floor in the 2 year old classroom, numerous pieces of trash throughout the floor on the 4 year old classroom while on the playground playing.
