A written activity plan was not posted or available for review in the infants, toddlers, and pre-k classrooms. This was corrected when the director provided copies of the plans to each classrooms and they were posted.

Due to the classrooms recently being remodeld, the emergency evacuation and relocation diagram was not posted in two of the classrooms. This was corrected when the director posted the diagrams in the classrooms.

An 8 year old was observed unsupervised in the front office. She exited the office and then went into the hallway alone, then a caregiver approached her. The child had left her classroom through a door shared with the front office to look for her mother who is also an employee.

Four caregivers are missing training in the following topics: -Handling, Storing, & Disposing of Hazardous materials -Preventing and responding to emergencies due to food or an allergic reaction -Adminstering Medication

Two caregivers who care for infants are missing current training in the following topics: Shaken Baby Syndrome, SIDS, and Early Childhood Brain Development.

The operation did not have documentation of monthly inspections of active play space for the months of January - July 2022.
