During the investigation, it was discovered that CCR was not notified within the required timeframe of 48 hours. Operation was made aware that child would be taken for medical treatment.

The director is missing 24.5 annual training hours from 2021 to include child development, discipline and guidance, curriculum, teacher-child interactions, serving children with special needs, abuse and neglect, emergency preparedness, preventing the spread of communicable diseases, administering medication, preventing and responding to food or allergic reactions, physical safety and hazards, handling hazardous materials, and three hours management.

Three of four staff files audited did not have current CPR training.

A caregiver grabbed a child by the forearm when the child refused to nap, resulting in two fingernail abrasions that broke the skin.

One preshool classroom had missing electrical safety outlets. This was corrected at inspection when director added to the safety plugs to the electrical outlet.

In one of the preschool classroom an electrical fan was unmounted and in easy access to the children. This was corrected at inspection when caregiver removed the fan and placed it in the closet.
