An outlet in the toddler room did not have a cover and was accessible to children. This was corrected at inspection when an outlet cover was installed.

A caregiver did not know the age of the youngest child in the group. This was corrected at inspection when the caregiver was given the information.

A three-year-old child was placed in a highchair for hitting and biting their teacher. This was corrected when the child was removed from the highchair.

An eight month old infant was observed sleeping in a restrictive device. This was corrected when the child was immediately removed from the restrictive device.

A child was observed in a high chair without using the safety straps. This was corrected at inspection when the child was removed from the highchair.

A person who received an ineligible letter due to not completing the background check process was present at the operation and in the presence of children.

The director did not use form 7259 to audit personnel files within 3 months of start of plan of action. A new caregiver did not complete Personnel Information form 2947 as required by plan of action.

The director has not been completing weekly classroom observations consistently as required under the operation's plan of action. Only three observations were available for review.

One child in care does not have immunization records on file.

Pre service training was not available for review for two caregivers.

First aid, CPR, and preservice training records were not available for review during the inspection.

Based on the information received 4 caregivers stated that they had not received CPR training. Furthermore, training records were not available to see if any additional caregivers had the required training.

During the inspection substitutions were made for lunch which were documented on a white erase board however breakfast substitutions were not recorded. According to the menu, pancakes and waffles were to be served for breakfast, however some children were served cereal, milk, and fruit.

Operational polices were not available for review.

Based on the information obtained four caregivers stated that they had not received first aid training.

The menu was posted in the kitchen and not in a prominent place accessible to parents.

The children did not use soap and water to wash hands before eating their meals. They cleaned their hands with liquid sanitizer.

A mop bucket filled with water was left unattended in the corner of a classroom and was accessible to children. This was corrected when it was addressed with the caregiver and the bucket was moved out of reach from children.

Two toddlers were observed alone in a classroom without appropriate supervision for less than 5 minutes. The door to the classroom was open. This was corrected when it was addressed with the caregiver and she took the children out of the room to join the rest of her class.

A 13 month old was combined with children 18 months and older during naptime. This was corrected when the caregiver moved with the infant into the infant room.

When asked for a child's name, the teacher mistaken the child for another child. When asked for the age of another child, the teacher stated they didn't know the child's age because they are new to care.

Two personnel files are missing notarized affidavits on file.

The director is lacking 8 hours of annual training for 2020-2021.

Two children are missing a signed health statement from their doctor after being in care for at least 12 months.

A classroom with 13 children was observed out of ratio during lunch time. This was corrected when another caregiver came into the room.

2 out of the 8 children's records evaluated were missing immunizations records on file. 3 out of the 8 children's records evaluated needed current immunization records on file.

Two children in the infant room were observed unattended during naptime. This was corrected when a teacher went into the clasroom after changing another child's diaper.
