Young children were observed eating crackers off a bare table top. The snack was not served on proper plates or napkins.

A caregiver did not have current training in the following topics: (1) SIDS, Shaken Baby Syndrome, Early Brain Development (2) Child Maltreatment (3) Emergency Preparedness (4) Communicable Diseases (5) Allergic Reactions (6) Building and physical Premises Safety (7) Handling, Disposing Hazardous materials

The hand washing sink in the diaper-changing area in the infant room was being used to also wash plates, utensils, and bottles. This was corrected when one basin of the sink was designated hand washing only and the other basin was designated for dish washing only.

A volunteer who has been counted in the child/caregiver ratio has not completed at least 8 hours of pre-service training.

Based on the evidence gathered, a caregiver did not use a positive method of discipline when the caregiver was observed tapping a toddler's bottom to redirect them off a table.

Three out of the ten childrens records reviewed were missing health statements on the required admission forms.

6 out of the 10 files reviewed were missing current annual training hours.

An infant was observed asleep in a bouncer. This was corrected when the caregiver was asked by licensing to move the sleeping infant to a crib.

One of the ten childrens records reviewed was missing emergency authorization.

Five out of the ten children's records reviewed were missing current immunization records.

Room 45 was observed out of ratio by 2 children. The specified age group was 2 years old and there were 13 children with one caregiver. This was corrected at inspection when 2 children were moved to another group.

Based on evidence gathered, on at least two occasions, an infant room was out of child/caregiver ratio. On 11/09/23, a caregiver was out of ratio by 3 infants. On 11/06/23, a caregiver was out of ratio by 2 infants. Based off the specified age group, the maximum number of children one caregiver may supervise is 4.

Based on information gathered, parents have not been allowed to enter their child's classroom upon request.

The documentation of fire drills and equipment checks was unavailable; it could not be located. This was corrected at inspection when a fire drill was conducted and equipment was checked.

The parent handbook did not contain procedure for parent notifications. The operational policies were also missing the promotion of indoor/outdoor physical activities, procedures for parental notifications, and procedures for parent participation in operation's activities.

One of thirteen employee files did not have a current First Aid certification in her file.

An employee who was present and in a classroom, was showing inactivated in the people's list. Note: This was corrected during inspection when the employee went home and was reminded to only return when she is cleared.

One of six children's files did not have an updated shot record in the file.

The infant classroom was observed with 5 infants under 12 months in care. NOTEE: This was corrected during inspection when a caregiver became free to move into the infant classroom aa a second caregiver.

During the investigation, it was determined that positive methods are not used as required discipline measures.

Two of six files reviewed did not have current CPR cards.

1. A review of the Employees Who Left Employment in 2021 data from February 23, 2022, revealed that the center had not reported the number of employees who left employment in 2021 by February 22, 2022, as required.
