The hand washing sink in the diaper-changing area in the infant room was being used to also wash plates, utensils, and bottles. This was corrected when one basin of the sink was designated hand washing only and the other basin was designated for dish washing only.

Based on the evidence gathered, a caregiver did not use a positive method of discipline when the caregiver was observed tapping a toddler's bottom to redirect them off a table.
