In the kinder readiness classroom, there were two cleaning solutions, toilet bowl scrubber and plunger under the sink cabinet accessible to the children as the cabinet didn?t have a latch. NOTE: THIS WAS CORRECTED AT INSPECTION.
Based on the information gathered, and interviews conducted the center treated the infant classroom by setting off foggers to get rid of roaches. NOTE: This was corrected as the center was treated by a pest company on 5.22.23.
While in the two year old classroom the children were provided breakfast before washing hands. NOTE: This was corrected at inspection.

Based upon information gathered as a result of this investigation and interviews conducted it was determined an employee at the operation displayed poor judgment, by passing a child through an open window in the mobile toddler classroom to their parent standing in the parking lot.
The operational policy did not specify the benefits of physical activity and outdoor play. NOTE: This was corrected at inspection when a written policy was included into the operational policy.
Based on the interviews and information gathered by administration, staff and parents related to this investigation there is evidence to support toddlers are watching television.
During an investigation it was determined that the caregiver did not properly conduct a head count when re-entering the classroom from outside play, resulting in her being unaware that a child was missing from the group.
During an investigation it was determined that the playground fencing was in a state of disrepair that threatened the safety of children due to a child being able to crawl under the gate.
During the course of the investigation neglectful supervision was determined to have occurred after a two year child crawled under the playground fence without the knowledge of any teacher or staff and was later found by a passerby on the sidewalk near a busy street. The operation and caregiver had no knowledge that a child was missing until notified by law enforcement.
On the changing table of the infant classroom cleaning supplies were accessible to children. NOTED: This was corrected at inspection when the cleaning supplies were removed to the sink counter.
