In the kinder readiness classroom, there were two cleaning solutions, toilet bowl scrubber and plunger under the sink cabinet accessible to the children as the cabinet didn?t have a latch. NOTE: THIS WAS CORRECTED AT INSPECTION.

Based on the information gathered, and interviews conducted the center treated the infant classroom by setting off foggers to get rid of roaches. NOTE: This was corrected as the center was treated by a pest company on 5.22.23.

While in the two year old classroom the children were provided breakfast before washing hands. NOTE: This was corrected at inspection.

An employee who no longer is with the operation since 4.15.2023 was still active on the people list. NOTE: This was corrected at inspection when the people list was validated.

One of the ten children files did not have special care statement available for review. Two of the ten children files did not have address for the emergency contacts. One of the ten children files did not have school-age information permission available for review.

Diapers were observed on the changing tables in the infant and both toddler classrooms. In addition to a diaper cream observed on the changing table in the infant classroom. NOTE: This was corrected at inspection.

The emergency preparedness plan did not specify procedures for evacuating infants under 24 months of age, who have limited mobility, or who otherwise may need assistance in an emergency, such as children who have mental, visual, or hearing.

The operational policy did not specify written requirements or procedures regards to water activity.

The April menu was not observed posted in the classrooms or the kitchen. NOTE: This was corrected at inspection.

Annual trainings of 24 hours for 2021-2022 were unavailable for review.

Based on the inspection into the infant classroom one caregiver was observed with five children with a specified age of 0-11 months. NOTE: This was corrected at inspection when an additional person went in to cover.

Based on the information gathered the operation failed to notify the parent at pick up when a child was bite on the back.

Based upon information gathered as a result of this investigation and interviews conducted it was determined an employee at the operation displayed poor judgment, by passing a child through an open window in the mobile toddler classroom to their parent standing in the parking lot.

The operational policy did not specify the benefits of physical activity and outdoor play. NOTE: This was corrected at inspection when a written policy was included into the operational policy.

Based on the interviews and information gathered by administration, staff and parents related to this investigation there is evidence to support toddlers are watching television.

During an investigation it was determined that the caregiver did not properly conduct a head count when re-entering the classroom from outside play, resulting in her being unaware that a child was missing from the group.

During an investigation it was determined that the playground fencing was in a state of disrepair that threatened the safety of children due to a child being able to crawl under the gate.

During the course of the investigation neglectful supervision was determined to have occurred after a two year child crawled under the playground fence without the knowledge of any teacher or staff and was later found by a passerby on the sidewalk near a busy street. The operation and caregiver had no knowledge that a child was missing until notified by law enforcement.

On the changing table of the infant classroom cleaning supplies were accessible to children. NOTED: This was corrected at inspection when the cleaning supplies were removed to the sink counter.
