In a room with a ratio of one caregiver to four children, a caregiver was alone with five children. Noted: This was corrected immediately when another caregiver was brought into the room.
Two cribs with children under 12 months had a stuffed animal in the crib while there was an infant using the crib. Noted: This was corrected immediatly when the toys were removed from the crib.
Two infants were found to be sleeping in restrictive devices. Noted: This was corrected at time of the inspection, when the infants were removed from the restrictive devices and places on their backs in their cribs.
The director did not have a completed training in administering medications or responding to emergencies due to food or allergic reactions.
The electrical cord to a bottle warmer was hanging down over the sink, accessible to infants. Noted: This was corrected at inspection when the cord was moved out of reach of the infants.
Five of the reviewed children's files did not have maintained vaccination records included in the file.
Information gathered for this investigation determined a staff member inappropriately disciplined their own child while at the operation. Staff member slapped the child on the side of their face and spanked the child?s bottom. Staff member also yelled at the child.
A caregiver placed a non-walking infant on a lawn chair resulting in the infant to lean forward and fall off the chair. The infant hit their face on the ground.
A bottle filled with water gel beads did not have the cover secured posing a chocking hazard in the infant II classroom .
Caregiver in toddler classroom sprayed the hands of a toddler with soapy water while toddler was laying on the diaper changing table. The cleaning solution sting the child's eye.
During an investigation it was reported that a caregiver expressed frustration and used inappropriate language towards children. Note: The deficiency was corrected at inspection / investigation when the Director made the decision to release caregiver of her duties.
This standard was reviewed in relation to the allegation and found to be deficient as the child's diaper had not been checked or changed for more than three hours.
During the investigation, it was found that a caregiver administered a prescription cream to the sibling of the child who it was prescribed to without a proper prescription at the parents' request.
