A child was observed in a restrictive device for 30 minutes. Note: This was corrected at inspection when the child was removed from the bouncer.

After diapering, a caregiver was observed to not wash their hands. Noted: Employee washed hands once it was brought the her attention.

The composite structure of the playground was observed to have a shelf and another climbing structure in the use zones. Noted: This was corrected at inspection when the equipment was removed out of the use zones and given its own use zones.

A classroom was observed to be out of ratio when a group of children with a specified age group of 5 years old was observed to have 23 children with one caregiver. The maximum group size for one caregiver for this specified age group is one caregiver to 22 children. Note: The operation moved two children to a different classroom, and there were two children that were picked up for the day.

It was determined that there was a child that was left on the van for 15 minutes.

It was determined that the electronic child safety alarm in the van, that was used to transport children, was not properly working.

It was determined that proper procedures for unloading the van were not completed, after a child was left in the vehicle unattended.

It was determined that a caregiver neglectfully supervised a child when the child was left on the transportation vehicle for 15 minutes.

There were multiple tears in the diaper changing table located in the infant classroom.

Three children files did not have current immunization.

The file for the infant room caregiver was reviewed during inspection and was missing training on SID's and Shaken Baby Syndrome.

The child safety alarm in van #1 did not sound off when the van was turned off. The child safety alarm beeps when the vehicle is turned on, but not when turned off.
