Caregivers had 18 children in care, the youngest being 16 months old and the oldest child in the group is an 8 year old, therefore the youngest/oldest children present are more than 18 months apart.

At the time of the inspection, two children did not have immunization records for review and one child's records were not current.

During the inspection, an infant caregiver did not have training in the required infant safety topics.

During the walkthrough, there were multiple children sleeping with their milk cups.

During inspection caregiver stated that she had 15 children in care, and after counting it was determined caregiver had 16 children in care.

Recite; Caregiver in the duckie classroom had 17 children in care. Based on the specified age group caregiver can only have 15 children in her group. Caregiver in the lambs room walked out of the infant room, leaving caregiver out of ratio with 7 infants.

One classroom did not have a smoke detector.

During inspection it was observed that an infant was walking around drinking his bottle.

The lambs classroom was observed out of ratio, caregiver had 5 children ages 3 month, 5 month, 8 month, 8 month and 12 month. Based on the specified age group caregiver can only have 4 infants in her group. Caregiver in the duckie classroom had 19 children in care. Based on the specified age group caregiver can only have 11 children in her group. Note: Two caregiver's arrived to assist with ratios.

Dust can be seen accumilated by an air vent. Empty spider webs were observed in a playground structure.

Good judgement was not exersiced when a two year old child was strapped to a high chair placed on an inadequate small chair.

Wooden boards observed broken in the playscape, The slide on the same play structure was observed cracked resulting in a gap. Nails were observed protruding from the playscape.

Children observed did not wash their hands before breakfast. Note: This was corrected at inspection when other children were directed to wash hands before eating.

An infant was observed sitting in a baby walker. Note: This was CORRECTED AT INSPECTION when the walker was removed from the infant classroom.

A caregiver was observed caring for children without an eligible background check. A 16 year old was observed at the operation without an eligible background check.

Thick layers of dust built up were observed on vents through out the center.

A blanket was observed in the crib with a 10 month old.

Infant's individual storage space must be labeled either with the child's name, photo or symbol.

The operation had an outstanding balance of $4 for their background checks and it was paid during insepction.

Two five years old were observed transported by the operation staff without a booster seat.

Based on the evidence gathered during the investigation, a three year old was transported without the proper front facing child safety seat.

A cord to the ac mounted in the window located in the three to five year old classroom was observed accessible to the children in care.

A hole in the ceiling and mold were observed in a closet located in the three to five year old classroom. Blinds in the three to five year old classroom were observed dirty.
