There was a trash can in the toddler classroom that had no lid. There was garbage from that mornings breakfast in the trash can. Additionally there were trash cans located in the bathrooms that had soiled diapers thrown in them, that had no lids on.

There was no documentation recorded of the carbon monoxide detection systems, smoke alarms, and fire extinguishers being checked monthly. The last date that was recorded was 3/16/2022 that these systems were last checked.
There was no lid on the trash can located in the toddler room. Note: The director placed a lid on the trash can during the inspection.

There was a piece of playground equipment that had broken. These broken pieces appeared to be sharp.
There was an infant that was sitting up with a bottle propped up by a pillow with a towel on top of it. Note: This was quickly corrected at inspection when the caregiver picked up the bottle and started feeding the infant.
It was determined that a classroom with a specified age group of 2 years old had 13 children with 1 caregiver. The caregiver to child ratio for this specified age group is 1 caregiver to 11 children.
It was determined that a caregiver failed to adequality supervise the children in care, when a child was able to walk out of the playground gate onto a busy street.
A two year old child was able to leave the playground area without the caregivers knowledge. The child was gone for approximately 15 minutes. The caregiver only realized what happened when the child was returned to her care.
The director did not ensure the operation was in compliance with minimum standard rules when a caregiver was left out of ratio, a playground gate was not checked to ensure it was properly closed allowing a child to leave and by allowing caregivers to be in direct care of children for more than 10 hours a day.
It was determined that caregivers were in direct care of children for more than 10 hours a day.
It was determined that the director did not inspect the playground area including the gate to the playground. The gate to the playground was unlocked and not latched which allowed a child to leave the playground space and walk to a busy street.
During the course of an investigation conducted by DFPS, it was determined that a 2 year old child was neglectfully supervised by a caregiver. This allowed the child to leave the playground area and walk into the busy street that the daycare is located on.
