The Toddler room had a loose cabinet knob and laminate that was peeling off of the counter. The infant room had a missing ceiling tile. There was a trash bag with old food by the fence on the open field side of the playground. A fence gate on the back playground next to the pool area has a gap over 5 inches from the ground to the bottom of the gate. A bathroom drain cover was not attached in the Prek classroom. An electrical control box for the door key pad off of the exit door to the back playground could be removed, exposing wires.

A caregiver was present and in direct supervision of children in care without a background check being entered or an eligibility letter on file. NOTE: Caregiver was sent home during the inspeciton and a background check was entered.
RECITE: A seatbelt on Vehicle #3 has not replaced the seat belt that was inoperable and covered it with red duct tape. The Facility Manager was present during the inspection and scheduled a mobile tech to come out on 5/14.
A child was observed sleeping with a sippy cup in their mouth. NOTE: The caregiver was asked to remove the cup during the inspection.
There was peeling paint on the wall and window sill in the Toddler room.
An outdoor outlet on the Toddler playground did not have a cover. The fence around the ac units on the back playground was curling allowing children to climb under or for clothing to catch on the fence. A utility knife was found open and sitting on top of a sensory table. NOTE: The knife was removed from the playground during the inspection.
The large playground climber measures 9 ft in height at the highest platform. NOTE: A waiver request was provided during the inspection.
A seatbelt on Vehicle #3 had a frayed seatbelt prohibiting it from being used.
