A caregiver left children unsupervised when they exited the classroom to answer the front door.

There was s swiffer wet jet mop in the infant room with a cleaning solution bottle by the wall close to the door.
The access door to the attic was detached from the ceiling in the infant classroom. The hinge on the door to the infant room was loose.
The caregiver in the toddler room was observed leaving children unsupervised in the classroom while attending the front door and when taking children to the restroom. The infant caregiver was also observed exiting the infant classroom to talk to the toddler caregiver.
One infant in care did not have an infant feeding schedule available for review. Another infant's feeding schedule had not been updated since March.
2 slides on the playground did not have a 6 ft use zone on one side of each slide.
A caregiver in the infant room did not wash the child's hands after a diaper change. This was CORRECTED AT INSPECTION when the infant was taken to the sink to have their hands washed.
A slide on the playground did not have 6 inches of loose-fill. The loose-fill at the exit of the slide measured at 1 inch.
A caregiver present and in the Pre-k classroom had a pending background check. Caregiver's time sheet showed a clock in for date 6/2/2023, 6/5/2023, 6/6/2023, 6/7/2023, 6/8/2023, and 6/12/2023. Note. Caregiver was ask to leave on 6/12/2023 and did not .
There was a tray with knives, pizza cutter and other kitchen utensils that were at hands reach of children in the kitchen. The kitchen is across the children's restroom and the kitchen door was open.
Re-cite, play equipment did not have the required loose-fill .
There were two play structures with slides that did not have loose-fill.
One child did not have immunization records on its file.

Children have not been going outside to play due to the issues with the fence.
The operation's fence has been moved by neighboring business resulting in large gaps underneath in which a child could fall down into the parking lot resulting in an injury.
Director is missing 2 required topics of training.
Evidence gathered during investigation showed a toddler holding a tortilla and sweeping the floor with it and then putting the tortilla in it's mouth.
