Three year old children were observed in a group of children with four infants (2, 5, 12, & 16 months). The three year olds were more than 18 months older than the youngest children in the group. This was corrected when a second caregiver arrived about 30 minutes later and the infants were moved to their assigned classroom.

The operation did not have infant feeding instructions on file for an infant in care. This was corrected when the director requested written feeding instructions from the parent.

One of the five staff files evaluated did not have a notarized employment affidavit on file.

An infant caregiver did not know the age of an infant in their care and did not have information in the classroom with the infant's date of birth or age.

6 infants were observed with 1 caregiver in the infant classroom. The specified age group was 12-17 months. This was corrected when the 18 month old in the infant room was moved to the toddler classroom, putting the group 1:5 and in compliance.

A caregiver did not have a notarized employment affidavit on file.

Cleaning solutions were observed in the cabinet underneath the children's handwashing sink, and the cabinet was not equipped with a child safety lock making them accessible to children. This was corrected when the solutions were removed from underneath the sink.

Playground maintenance checklists from the last 3 months were not available for review

Concerns with supervision were observed when the director stepped out of the classroom she was supervising to answer the front door and get snack from the kitchen. This resulted in the children being temporarily unattended.

Current written activity plans for infants, toddlers, and pre-k were unavailable for review during inspection.

Issues with supervision were observed during inspection when caregivers needed to step out of the classroom several times to get items from the kitchen, front office, and infant classroom leaving the classroom the children were in temporarily out of ratio.

An infant was observed sitting in a bouncer seat for more than 15 minutes at a time. This was corrected when it was addressed with caregivers and the infant was removed from the restrictive device.

The operation was out of ratio for less than 5 minutes when a caregiver was left alone with a group of 12 children including 3 infants while the second caregiver had to go outside to bring in an after school age child from the bus stop. This was corrected when the caregiver returned to the classroom.

The operation's most recent fire inspection report was not available for review.

Feeding instructions for two infants in care were not available for review.

Three children do not have current immunization records on file.
