There was a dead cricket in the dining hall under the rolling cart. There were dead cockroaches and crickets on the floor with collected debris. There was a spider with its spider web on the windowsill, dust and dead cockroach.

The room with the cribs was dark during naptime and had two infants sleeping in their cribs. Note. This was corrected when the caregiver opened the curtain to allow light in the room.

There was a hole on the wall exposing electrical wires to an outlet. This poses a risk to any infant, but especially the one who is laid down on a crib within hands reach.
The operation recently had their chain link fence replaced, leaving several areas with at least a 10-inch gap from ground to fence.
The restrooms located in the hallway did not have soap in the dispensers. Note. This was corrected at inspection when the soap dispenser was refilled.
The trash can in classroom #5 had the lid removed and found inside the can. There was a trash bag filled with garbage that was not taken out on the last operating business day. Classroom #4 was observed with several different sizes of dead cockroaches on the table, tile floor and rugs. Note. This was corrected at inspection when the trash bag was removed and replaced with a new bag and lid placed over the can.
The toddler classroom for 18-23 month old children had magnetic Bingo chips designated for ages 3 and up. Note. Although the classroom has a combine group of children ages 2 -4 years old, the classroom is still considered and approved for the toddler children. Therefore, the toys and equipment must meet age appropriate guidelines.
At the time of the walk through inspection, a group of children in the infant classroom were observed having screen time. One child was two years old.
At the time of inspection, one infant did not have a current feeding log for the month of June. The infant went from drinking breast milk to formula.
A caregiver disregarded the feeding log a parent had recently updated for the month of April and served an 8-month-old infant cranberry juice. The juice was not listed on the feeding log.
An infant's feeding instruction was not followed when a staff served an infant cranberry juice.
