Based on the evidence gathered, the fence that encloses the playground had a hole in it large enough for a small child to fit through. This was corrected when the operation repaired the hole.

Three caregivers are missing annual training hours for training year 2022-2023.

Three children did not have current immunization records on file.

A child with a diagnosed food allergy did not have a food allergy emergency plan from the doctor on file.

A caregiver present at the operation did not have an active background check on file. This was corrected when the caregiver left the operation and a BGC request was submitted by the operation for this caregiver.

During inspection, a caregiver was heard telling a child "No,thats disgusting" in an ugly manner. . A child was heard crying immediately after comment was made. When director was asked about it, she said it was due to something the child did.

One of six children's files did not have a signed medical authorization. This same file did not have an address for the emergency contact.

The director did not have a current CPR/First Aid certification.

Two of six children's files did not include a health care statement.

Three of six caregivers had not competed their required 24 hours of annual training.

While reviewing childrens records, one of the four records did not have any emergency contact listed. Operation will obtain information for child's file on Monday 2/28/22 when parents return with child to operation. All have been sick and unable to return to center.

During inspection, the director did not produce daily log of maintenance for playground area, and stated this is not done frequently.

During inspection, the director could not provide a schedule of carbon monoxide texts conducted at operation.

Observed infant crib with a mattress that is propped up, information is not provided for positioning of mattress.

Observed 2 infants (14 months) sitting in highchairs and not securely fastened with safety straps. This was corrected when caregiver fastened safety straps on both children.

Observed the playground with high grass throughout that is not maintained. Observed a waterhose that was on the playground unraveled crossing the sidewalk leading to the playarea. This was corrected when caregiver removed the waterhose.

Observed a crib with a loose fitting sheet.
