During the inspection, a video was provided of the incident. The video had shown a toddler was left unattended on the elevator for 5 minutes until the toddler was found by the caregiver.

A video was reviewed pertaining to the incident that occurred on 7/21/2021. One caregiver was supervising five children in the sandbox, while one child was observed kicking and standing on another child's back. Another child was playing in the rock area unsupervised. The second caregiver was inside the classroom with another.
